First visit procedure 初めて受診される方

Before visiting our hospital for the first time, we recommend that you visit your previous doctor and obtain a referral including any treatment information. This is to help make your visit at Tsuchiura Kyodo General Hospital run more smoothly.

For patients visiting the hospital for the first time without a referral from another medical institution, a special fee of 11,000 yen for your first consultation will be charged. Otherwise, a special fee of 5,500 yen will be charged if your symptoms are stable and you visit our hospital even though you have offered to refer to another hospital or clinic in writing These fee will not be covered by your national health insurance plan.

初めて受診される場合は、できるだけお近くの医療機関を受診され、診療情報を含む紹介状をご持参の上受診して下さい。当院での診療がより円滑に進みます。
紹介状をお持ちでない場合でも受診いただけますが、医療費の他に初診時選定療養費として、11,000円を自費でお支払いいただく事になりますのでご了承ください。
また、症状が安定し、他病院または診療所等に対して文書による紹介を行う旨の申し出を行ったにもかかわらず、当院を受診した場合にも再診時選定療養費として5500円をご負担頂きます。

Upon your arrival, please fill out the medical examination application form and send it to the general reception desk.
Then If you have a letter of introduction from another medical institution, please submit it to the referral reception desk together with the application form.

診察申し込み書を記入して総合受付窓口にお出し下さい。
他医療機関からの紹介状をお持ちの方は、申し込み書と一緒に紹介受付へお出し下さい。

交通事故、仕事中のけが等で受診の方はあらかじめお申し出ください。
保険証の提示がない場合は、自費扱いとなります。ご注意ください。

Return visit 再来の方

Please use the return reception machine. If you have any questions, please contact the general reception.
In addition, we check your health insurance card once a month. Even if you are undergoing outpatient visits on a continuous basis, please bring your health insurance card to each department block counter on the first visit day after the month changes.

再来受付機にて受付をお願い致します。ご不明点等ございましたら総合受付までお問い合わせください。
なお、毎月１回健康保険証の確認をさせて頂いております。継続的に外来受診されている方でも月が変わって初めての受診日には健康保険証を各科ブロック窓口にお出しください。

Appointment for your next visit 次回の予約について

If you make an appointment for your next visit at the end of your consultation, you will receive a sheet indicating your visit schedule along with a detailed statement of your medical bill. Please bring the appointment sheet with you during your next visit.

医療費のお支払い後発行される領収書に次回の予約票がついています。
次回来院時に切り離してお持ちください。
